Booking Agreement

Thank you for choosing VivRant Health & MedSpa. By scheduling an appointment with us, you acknowledge and agree to the following terms.

Appointment Requests

Appointments may be scheduled online through our booking system or by contacting our office directly. Appointment requests are subject to provider availability and are not considered confirmed until you receive a confirmation.

Arrival Time

Please arrive 10–15 minutes early for your first appointment to complete any necessary paperwork. Returning patients should arrive a few minutes before their scheduled appointment.

New Patient Forms

New patients may be required to complete medical history and consent forms before receiving treatment. Delays in completing required forms may result in a shortened appointment or the need to reschedule.

Cancellation & Rescheduling

If you need to cancel or reschedule your appointment, we kindly ask that you provide at least 24 hours' noticewhenever possible.

Late cancellations or missed appointments may be subject to a cancellation or no-show fee.

Late Arrivals

Patients arriving late may have their appointment shortened or may need to be rescheduled to avoid delaying other scheduled appointments.

Deposits

Certain services may require a deposit at the time of booking. Deposits are applied toward your scheduled service and may be forfeited if the appointment is canceled outside of our cancellation policy or if you do not arrive for your appointment.

Treatment Eligibility

Some services require a consultation before treatment. Your provider will determine whether a treatment is appropriate based on your medical history, current health, and treatment goals.

For your safety, VivRant Health & MedSpa reserves the right to decline or postpone treatment if it is not medically appropriate.

Medical Information

You agree to provide complete and accurate medical information, including current medications, allergies, and medical conditions. Failure to disclose relevant health information may affect your treatment and its outcome.

Results

Every patient responds differently to treatment. While we strive to provide exceptional care, individual medical and aesthetic results cannot be guaranteed.

Payment

Payment is due at the time services are rendered unless otherwise specified. We accept approved forms of payment listed by our office.

Communication

By scheduling an appointment, you consent to receive appointment confirmations, reminders, and service-related communications by phone, email, or text message.

You may opt out of marketing communications at any time.

Emergency Situations

If you are experiencing a medical emergency, call 911 or go to the nearest emergency room. Our online booking system should not be used for urgent or emergency medical needs.

Agreement

By scheduling an appointment with VivRant Health & MedSpa, you acknowledge that you have read, understood, and agree to this Booking Agreement and our Privacy Policy and Terms & Conditions.
